Here’s How to Get Started

To become a customer of Phillips, print and complete the below applications.

Follow the simple 5 step process below to become a new Phillips Pet Food & Supplies customer. Please print and fill out the relevant forms. All printed forms must be signed where appropriate using blue or black ink.

Step 1: Customer Application

To become a customer of Phillips, please fill out the below application.

If you are applying as a veterinary office, please fill use this application instead.

Step 2: State Tax Exemption

Select the state your business is located in, then download and complete the relevant state tax form.

Step 3: Directions

Please provide delivery information and directions in order to assist Phillips transportation personnel in making timely non-evasive deliveries.

Please provide us with some information about your business.

Check to make sure you have completed all the forms and supplied any necessary documents.

  1. Print all forms and use a blue or black ink to sign where needed.
